Diagnosis

Initial management of trauma, follow ATLS

  1. Set-up prior to patient arrival to OR

    Assemble team and assign roles

  2. Estimate weight and prepare emergency drugs

  3. Gather equipment:

    • Airway supplies
    • Line placement and monitoring devices
    • Fluid warmer/rapid infusion device
    • Code cart with programmed defibrillator
  4. Type and cross blood products. Activate massive transfusion protocol if indicated

    • Call Blood Bank
  5. On patient arrival to OR

    Maintain c-spine precautions for transport

  6. Secure/confirm airway (often aspiration risk, unstable c-spine)

  7. Ensure adequate ventilation (maintain PIP < 20 cm H20)

  8. Obtain/confirm large-bore IV access (central or intraosseous if peripheral unsuccessful)

  9. Assess hemodynamic stability. Pre-induction fluid bolus recommended if hypovolemic

  10. 20 mL/kg LR or NS (repeat x 2) and/or 10 mL/kg RBCs or 20 mL/kg whole blood.

    • 20 mL/kg, repeat x 2
    • 10 mL/kg
    • 20 mL/kg
  11. Arterial and central venous line placement if indicated

  12. Maintain normothermia

  13. Monitor and treat associated conditions

    • Anemia, coagulopathy, acidosis, electrolyte derangements
  14. Continuously assess for undiagnosed secondary and/or developing injuries, blood loss

  15. Consider TXA 10-30 mg/kg IV bolus then 5-10 mg/kg/h IV infusion within 3 hours

    • Bolus 10-30 mg/kg IV
    • Then 5-10 mg/kg/h IV infusion within 3 hours
  16. Crisis Management (If Severe)

Notify surgeon, call for help and code cart

  17. Call for Transesophageal Echocardiography

  18. Start chest compressions

  19. Give epinephrine 10 MICROgrams/kg IV/IO

  20. If cardiac arrest, go to CARDIAC ARREST

  21. Consider ECMO

Patient weight

Between 0.5 kg and 120 kg

Reset the checklist?

Checked steps, the timer and this crisis log are cleared. The patient’s weight is kept.

Finish the crisis?

The timer pauses, and you can review the case and the event log.
